What we do
Practical and emotional support to young carers in transition from children's to adult services. The service provides information, support and guidance as well as ensuring young adult carers get an assessment of their needs. This is a partnership with Kirklees adult Carers Service. We work with young adult carers from 16 to 18 years and then support the transition to the adult carers service.
