St Helens Dynamic Keyworker Service

What we do

The Barnardo’s St Helens Dynamic Keyworker service supports children and young people up to the age of 25 who have a learning disability and/or autism. We support young people with the most complex needs who are in crisis, at risk of admission to a mental health inpatient unit or who are in hospital.

Allocation of a Keyworker can only be made through St Helens Transforming Care and Gateway Panel for those who are registered on the Dynamic Support Database (DSD).

Our Keyworkers aim to:

• Improve the young person’s lived experience so that they feel, happy and involved in their care and support.
• Reduce stress and uncertainty experienced by young people and their families.
• Be a trusted person who actively listens and understands the young people and families they are supporting.
• Agree desired outcomes with young people and families.
• Advocate for and provide direct support to the young person to enable their voice, wishes and feelings to be heard.
• Liaise with agencies involved to ensure the right services are in place at the right time and to identify gaps.
• Provide flexible person centred, needs based and age-appropriate support.
• Support coordination of services across health, education, social care and community-based resources.
