- ongoing programme of assessment

After Care Service
Regular community visits, including evening and weekend visits, focus on:
- ongoing assessment and support of childcare
- emotional support
- establishing links in the community
- exploring opportunities for education and employment
- practical support
- support for mothers who are separated from their children.
Visits are usually undertaken at times when families themselves have identified a need for extra support or when there are no other supports available. After Care visits can continue for several months after a residential placement at PACT has ended.
