What we do
By using a person-centred approach we develop programmes which meet the needs of the individual and encourage them to look forward and identify future opportunities.
We strive to provide continuity of support from primary school during the move to high school and thereafter when or if the need arises. In order to facilitate this, the project worker within a primary school will remain the key worker for identified young people requiring additional support into their first year of high school (S1), but will also work collaboratively with the identified high school worker should support beyond S1 be required.
- Full assessment carried out with young person and their family to identify most appropriate support package.
- Weekly meeting with identified project worker.
- Progress of work reviewed at regular intervals.
- Duration of support dictated by young person’s needs.
- Discussion with young person and their family around purpose of support.
- An issue based or peer-support approach toward resolving difficulties.
- Weekly sessions with identified project worker(s).
- Duration of group is around 8 weeks.
- Opportunity for a family to work together with identified project worker(s) to find solutions to difficulties they may be facing.
- Duration agreed between the family and identified worker(s).
In their final year at primary school pupils can access supports to alleviate any worries they may have about the move to high school. These supports are delivered in the following ways:
- groupwork support
- Talk Time – an opportunity to voice feelings, ideas and thoughts, to seek information or just have a chat in a confidential and safe environment
- individual support
- Aiming for High – a support programme with all P7 pupils who may be experience difficulties with the transition from Primary school looking at the skills and information needed for the move to high school
- ongoing support during the summer holidays and into S1 is available where necessary
