How does Manchester Leaving Care Service get to know about me?

Usually as you approach your 16th birthday in care, your Social Worker will discuss with you a referral to Manchester Leaving Care Service. They will fill in a referral form (with your help if possible) and send us some further background information telling us why you are in care in the first place, plus a copy of the most recent review and planning meeting outlining your present situation.

Though it shouldn't happen, sometimes young people slip through the net and aren't referred at 16. If this does happen, don't worry, you can be referred any time until your 21st birthday. Besides your Social Worker, any of these people can tell us about you:

  • You.
  • Your family or friends.
  • Your Foster Carer or Residential Worker.
  • Any other agency that knows about you.

Whoever tells us about you, we will expect you to know this is what's happening, and that preferably you yourself have had a say in what is said about you.
